Shelter Bilingual Case Manager

 

 

Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty.  The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty.  Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.  

 

Purpose: The case manager will assess each Stephen Center client to determine need for physical and mental health services, housing and employment assistance, services for children, or other support as required.  The Case manager will then work closely with the Shelter Manager and appropriate community agencies and services to provide the best possible solutions to each client’s challenges.  Documentation of all client assessments, services provided and outcomes will be essential to acceptable job performance.

 

Key Accountabilities:

·Ensure all safety and security guidelines are followed.

·Prepares individual case plans for new admits within one business day (24 hours) of intake, consulting with the managers and others as needed.

·Follows through with the implementation of the service plan, working with the client and the appropriate community services and agencies.  Formally documents all actions taken, client involvement, outcomes, problems, etc.

·Meets with the shelter manager to consult on client service plans and to keep them informed regarding client status.

·Manage the filing of all service plan records generated by coordinating the development of a functional record keeping system to serve each client as well as Stephen Center staff information and statistical reporting needs.

·Enter data into Clarity within 24 hours of service.

·When a case plan is successfully completed or the client leaves the Stephen Center, finalizes the service plan record and files for future reference.

·Develop an understanding of the roles and challenges of other Stephen Center staff members and key volunteers and the daily operation of the Shelter in order to work effectively within that framework.

·Document all client assessments, services provided and outcomes will be essential to acceptable job performance.

·Maintain clarity license

·Complete Coordinated Entry assessments and place clients in community que

·Other duties as assigned

Qualifications:

  • Bachelor’s degree in social work, human services, psychology, or a related field; equivalent experience may be considered.
  • Experience working with individuals experiencing homelessness, addiction, or mental health challenges preferred.
  • Strong interpersonal, communication, and organizational skills.
  • Demonstrated ability to build rapport with diverse populations, including individuals in crisis.
  • Knowledge of community resources and referral processes in the Omaha area is a plus.

Position Requirements:

·Bilingual in Spanish/English

·Human Services, Social Work or related degree preferred/ or high school diploma and 2 years’ experience.
