A BIT ABOUT US

Founded in 1875, our agency has grown over the years to address most of the challenges children, adults, families, and communities face across the lifespan, including substance use, child abuse and neglect, domestic violence/sexual assault, human trafficking, homelessness, juvenile justice, mental health, neighborhood distress, and poverty. Today, we offer 42 programs from 20 locations in east central Nebraska and southwest Iowa. THE OPPORTUNITY

Diverse case management responsibilities that include assessments, person-centered care planning, working directly with member to assess holistic health care and social service needs; member strengths, abilities, and desires, assisting clients in addressing barriers by utilizing community resources and health related information; coordinating services, resources; working within a team model; and trainings opportunities. Ability to work with a diverse population of clients with co-occurring disorders utilizing evidenced based practices. Encouragement to explore your passions and areas of specialization and explore opportunities to grow within the agency.

Compensation: between $19.74 and $23.54 per hour (wage is based on total years of relevant experience)
Work Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m. (37.5 hours per week)

WHAT YOU’LL BRING

You could be perfect for this opportunity if you’ve had at least two years previous related experience and a bachelor’s degree in social work or related field. We are looking for someone who can build strong rapport with diverse groups, and who is exceptionally reliable, organized, detailed-oriented, and takes a trauma informed approach to engaging with clients.  If you have a desire to work in a fast paced, collaborative, team-oriented environment, then Heartland Family Service is the agency for you.

THE SKILL SET

      Within 3 months, you’ll:

  • Complete onboarding program designed to get you up to speed on our business, vision, departments, programming, and team and gain an understanding about how your role fits into Heartland Family Service’s broader organization mission and strategy.
  • Attend new employee orientation training to learn the mission of the organization, how we are doing, where we are going, and our overall agency philosophy.
  • Work with team and manager to understand work expectations and set goals for the first three months in the role
  • Get to know our policies, procedures, and clinical tasks and processes
  • Be introduced to and ramped to use Evolve (electronic health record).
  • Partner with manager to define key success metrics for your role and how to keep track of your progress
  • Weekly supervisions with supervisor.

3- 6 months, you’ll:

  • Develop full cycle knowledge in leading and supporting core processes independently
  • Develop a case load
  • Continue to learn about programs across the agency
  • Network with partner agencies and others in the community
  • Increase knowledge of skills, theory, and practice

6 -12 months onwards, you’ll:

  • Increase of training opportunities
  • Identification of goals to build areas of specialization and passion
